Referrals
Patients must be referred by a physician. A referral form can be downloaded here or physicians may use their own referrals. All referral forms must include the following:
- Medication and medical history. List current medications or attach a medication profile including non-prescription and herbal medications. Please include any relevant medical history (cardiac, renal etc.) and test results.
- If patient has x-rays, send the x-rays with the patient. Only send x-rays that have been taken within the last 3 months and include the following views:
- Knee- Standing AP, lateral and skyline
- Hip- Ortho pelvis, AP and lateral of affected hip
- If patient does not have x-rays, they will be done at the patient appointment.

- We will contact your office and the patient with the appointment details.
