Referral Process

Patients must be referred by a physician. Please make sure patients have had the necessary x-rays completed and that they have been taken within the last 6 months. The specific x-rays required are listed on the referral form. Please have patients bring the x-rays with them to their first appointment.

The Regional Joint Assessment Program supports a chronic disease management model for osteoarthritis for patients with lower extremity joint problems involving the hip and knee. 

The health team:

• Physician

• Advanced Physiotherapist Practitioner

	Dr. Woolfrey- Orthopaedic Surgeon	Dr. Dill- Orthopaedic Surgeon

 

The inter-professional health team assesses the patient and assists the patient in determining the best personalized treatment plan which may include non-surgical choices and/or surgical choices up to and including joint replacement surgery.
